- General health care is for all persons with disabilities; primary disability and integrated management is for persons with severe physical, brain lesion, visual, intellectual, mental and autism disabilities. Choose a doctor registered as a health doctor to receive overall health care and primary disability management. Home-visit rehabilitation service newly introduced from 2026.7.1.

- Open all year (apply by visiting a medical institution participating in the pilot program)
- How to apply
